Also, I don't like rule #2 that much; if someone gets an answer wrong and someone else knows the correct answer, they should be allowed to correct it.

Actually (I had to look this up), Fan Rotom's secondary type is Flying, and it keeps its Levitate ability regardless of form, so Rotom fits the bill, sometimes.

The pattern on this pokemon's belly is a mirror image of the pattern its evolved form has.

This species is known for being very weak until it evolves, knowing only Splash until it is over half the way to evolution.

Edit: Sorry, I didn't notice that BrandedOne answered my question. Tentacruel AND Octillery are correct. Tentacruel resembles OoT, MM, WW octoroks, and Gambler's answer, Octillery resembles the original artwork of an Octorok. So both are correct :)
